Addressing the Challenges of Poverty national conference presentation spotlight

Nurse practitioner Barbara Wise has used Bridges in her practice with under-resourced patients for 20 years. Barbara and Terie Dreussi-Smith recently published a scholarly paper linking Bridges, population health, and health practice. Here is a close-up view of Bridges skill sets at the most professionally intimate level—the typical office visit. Participants will also codesign impact step sheets for providers and patients.

Learning objectives include:

  • Review the targeted Bridges to Health and Healthcare insights and strategies Wise developed from the individual lens of a healthcare practitioner.
  • Engage in a small-group exercise to co-design and build Impact steps sheets for healthcare providers and patients.
  • Analyze and discuss the efficacy of using the Bridges to Health and Healthcare lens and skill sets as a practitioner or Bridges health advocate.
